MEDICAL FORMS
Each participant must complete a BSA Medical Form Parts A/B/C, including a physical. Medical forms due June 1, 2026.
ummer Camp Medical Forms along with a copy of your scouts Medical Insurance Card (front and back) are due by Monday, June 1, 2026, scout meeting.
This includes Scouting America Annual Medical Forms A, B1, B2 and C (school or sports forms cannot be uses as a substitution).
Tips for filling out these forms...
Form A: The participant signature line needs to be signed by the Scout and then the Parent/guardian will sign the next line. Except in the case that this form is for an Adult then it will only be signed on the Participant line.
Form B2: Check the box if there are no medications taken routinely, or fill in what Medications are taken in the box provided, including any over-the-counter medications.
Below that are two boxes (Yes or No) for Non-Prescription Medications – this is the area that given us permission to give a scout any over-the-counter medications, like Tylenol, Motrin, Benadryl etc…. at a scouting event if needed. Parent/Guardian then needs to sign on the line below.
If Medications are being given routinely then your doctor must also sign next to the Parent/Guardian on Form B, as well as on Form C.
Immunizations listed must include a Tetanus Shot dated within the last 10 years to attend Summer Camp.
Form C: This form is Required if you are going to attend Summer Camp or any other Scout/Leader Training. This form must be filled out and signed by a MD, DO, NP or PA with in the last 12 months, along with the doctor signing on Form B2 if any Medications are being taken routinely.
Medications to be given at camp: all prescriptions and over-the-counter medications must be brought in their original containers with clear dosing instructions and turned in to be administered by the scout leader or camp staff that is in charge of administering medications to the scouts, with the exception of rescue inhalers for such conditions like asthma. Those will be carried by the scout for use as need will at camp.
